Elevance, investment firm roll out Mosaic Health primary care initiative

Elevance Health Inc. announced the new Mosaic Health, “a national care delivery platform” dedicated to primary care.

The company, which changed its name from Anthem in 2022, joined with investment firm Clayton, Dubilier & Rice (CD&R), and the medical groups Apree Health and Millennium Physician Group to roll out the primary care initiative first announced in April. Mosaic Health has a new website saying the company is “focused on expanding access to comprehensive primary care for consumers” with various types of health care coverage.

"Mosaic Health will innovate on existing risk-based care delivery models, and I am excited to work closely with its operating companies and Elevance Health to foster collaboration and help Mosaic Health's operating companies better serve our providers and deliver exceptional care and services to more patients and communities," CD&R Operating Partner Clay Richards said in a news release. Richards will serve as Mosaic Health executive Chairman.

"Mosaic Health demonstrates CD&R's continued commitment to investing in and growing innovative health care companies that increase access to high-quality and affordable healthcare, and we are excited for this new chapter of innovation, expanded access and growth," Richards said.

Apree Health uses a value-based care model for advanced primary care, partnering with “top U.S. employers, leading provider groups, and some of the country’s largest, most progressive health plans,” according to its website. It also uses Apree Navigation for integrated care navigation and reduced benefits complexity, and Apree Advocacy for higher risk patient populations.

Millennium Physician Group started in 2008 and has grown to become one of Florida’s largest comprehensive primary care practices, with more than 750 physicians and other health care providers at more than 300 locations across Florida, North Carolina and Texas. The company also has more than 30 specialty services and employs phlebotomists, physician assistants, nurses, physical therapists, diabetes educators and other support staff, according to its website. The CD&R website includes Millennium Physician Group among its portfolio of investments.

Joined to form Mosaic Health, the business units will serve nearly 1 million patients across 19 states. The health care platform will accept payment through commercial insurance, Medicare, Medicaid and insurance provided through the exchanges created by the Affordable Care Act.

"Mosaic Health represents a new model of advanced primary care, and we look forward to collaborating with Mosaic Health and CD&R to accelerate growth in new markets and deliver outstanding care to more patients and communities across the country," said Bryony Winn, president of Carelon Health at Elevance Health. Carelon Health was formed in 2022 as a payer-agnostic subsidiary of Elevance Health, and it now serves one in three people across 50 states with more than 40,000 workers, according to the Carelon Health website. Carelon Health has divisions including CarelonRx and Carelon Services, with Digital Platforms, Health, Behavioral Health, Global Solutions and Insights, devoted to various aspects of health care.

In the second quarter, Carelon Health had $13.3 billion in operating revenue, up $1.2 billion from the same quarter in 2023 due to growth and the acquisition of Paragon Healthcare in the first quarter, according to Elevance’s second-quarter report.

Overall Elevance operating revenue was $43.2 billion in the second quarter, down $200 million from the second quarter 2023, due to attrition in Medicaid membership, according to the company.
